Right Inguinal Hernia

A nineteen-year-old male came to me along with his mother. He had noticed a right groin swelling a few days back. He had no pain, vomiting, constipation. On examination, there was a painless swelling at right groin. Usually such swellings can be pressed to reduce its contents and the swelling disappears. In this case, the swelling could not be reduced. Ultrasound examination showed swelling containing fat – omentum (pad of fat inside the abdomen covering the intestines, can enter the hernial defect and grow over a period and thus getting stuck to the hernial walls). The need for surgery was explained to them. The options of open and laparoscopic surgery were discussed with the patient and his parents. He was fit to undergo general anasthesia. After discussing the risk factors, a decision was taken to plan a laparoscopic hernia repair surgery. During the surgery, the defect was identified and the omentum was seen entering the hernial sac. This was reduced with care to avoid tear and bleeding from the omentum. The pre-peritoneal space was created to place a polypropylene mesh. The procedure was completed laparoscopically. He recovered uneventfully – was discharged on the third postoperative day.
